Handicap Parking Permits
You need this form filled out and certified by a licensed Physician, Osteopathic or Podiatric Physician, Chiropractor, Optometrist, or an Advanced Registered Nurse Practitioner from a facility operated by the US Department of Veterans Affairs or a Physician's Assistant from a facility operated by the US Department of Veterans Affairs. You bring it to our office with a valid Florida ID or Drivers License and we can issue you parking placard.

What is the cost? What forms of payment do you take?
Cost is $ 3.50 for a permanent and $ 17.50 for a temporary placard.
Do I need a Florida Drivers License or ID? What if I cannot walk anymore and cannot get one.
You do. There is no way for us to issue a handicap parking permit without valid Florida ID. Call our nearest Driver License office at 305 229 6333 for information on how to get a license and to make an appointment.
Do you issue Florida ID's or Drivers Licenses?
No, you have to go to a Drivers License office. A list of offices you can find here: here
We strongly recommend you call first for an appointment.
Do you issue stroller parking permits?
No, you can get them at the county main office at 140 West Flagler Street, Ground Floor, Miami.
Do I need to go to a Florida physician to have my handicap certified?
No, as long as the physician is licensed to practice medicine in the United States, it is acceptable
